- Lost certificates cannot be reissued.
- The Hellenic American Union can issue a certification of loss of a certificate for use in Greece. The service is charged with administrative costs. Please click here to check the cost.
To proceed with the issuance of a Certification of loss or damage of a certificate, please send or submit in person (Massalias 22, 10680, Athens, 9:00-17:00) the following:
- The relevant application form completed and signed.
- Declaration Form of Law 1599/1986. Verify the authenticity of your signature on the form at a Police Station or Citizen Service Center (ΚΕΠ). Alternatively, you can issue the Solemn Declaration form electronically through gov.gr with a digital authenticity of signature.
- A copy of your identity card or passport or any other legal document of identification (driver’s license or certification of identification ‘’tautoprosopia” containing a photograph stamped by a Public Authority).
- A copy of the proof of payment (in case of shipment of documents).
Please note that for the collection of the certification of loss by:
- First-degree relative: It is necessary to present all the above along with the recipient's ID
- Any third party: It is necessary to present all the above along with the recipient's ID and authorization to act on your behalf via gov.gr
